HIPAA Notice of Privacy Practices

Important Notice

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. Aevum Zenth Conglomerate, through its Zenth Health Sciences division, is committed to protecting the privacy of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable state and federal regulations.

How We May Use and Disclose Your Health Information

We may use and disclose your PHI for the following purposes:

• Treatment: We may use and disclose your health information to doctors, nurses, technicians, and other healthcare personnel involved in your care to provide, coordinate, or manage your treatment and related services.
• Payment: We may use and disclose information to bill and collect payment from you, your insurance plan, or other third parties.
• Healthcare Operations: We may use and disclose information to support the operational functions of Zenth Health Sciences, including quality assessment, staff performance reviews, training of students, and administrative compliance activities.
• Marketing & Fundraising: We will not use or disclose PHI for marketing purposes without your written authorization. We may contact you with general health-related fundraising communications, which you may opt out of at any time.
• As Required by Law: We will disclose information when required by federal, state, or local law, such as reporting suspected abuse, neglect, or domestic violence, or for public health activities.

Your Rights Regarding Your Health Information

Under HIPAA, you have the following rights regarding your protected health information:

• Right to Request Restrictions: You may ask us not to use or disclose certain PHI for treatment, payment, or operations. We are not required to agree to all restrictions unless the information is related to a service you paid for in full out-of-pocket.
• Right to Request Confidential Communications: You may request that we contact you in a specific way or at a particular location (e.g., home phone instead of work).
• Right to Inspect and Copy: You have the right to review and obtain a copy of your medical and billing records, subject to limited exceptions. Requests should be submitted in writing and may be subject to a reasonable, cost-based fee.
• Right to Amend: If you believe information in your record is incorrect or incomplete, you may request an amendment within six years of creation.
• Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI for purposes other than treatment, payment, or operations, spanning up to six years.
• Right to a Copy of This Notice: You may request a printed copy of this Notice of Privacy Practices at any time by contacting our Privacy Office.

Our Obligations

Aevum Zenth Conglomerate is legally required to:

• Maintain the privacy of your protected health information.
• Provide you with this notice of our legal duties and privacy practices with respect to your PHI.
• Comply with the terms of this notice currently in effect.
• Notify you promptly following a breach of unsecured protected health information, as required by the HITECH Act.
